**REMINDER**

Get your Kindy application in by the end of term (Thursday 23rd September) to secure a spot for 2022.

If your child was born between 1st July 2017 and 30th June 2018, you can come and pick up an Application for Enrolment from our front office or download one online from our school website: https://www.mandurahps.wa.edu.au/…/ApplicationforEnrolm…

When submitting your application, you will need to provide the following supporting documents:

* Your child’s Birth Certificate

* A copy of your child’s Immunisation History Statement (Available through MyGov)

* Proof of your residential address (e.g lease agreement or a utility

bill)

*Any relevant VISA information if applicable (e.g. VEVO Record)
